Treatment of acute pancreatitis with protease inhibitors: a meta-analysis.
"OBJECTIVES: Protease inhibitors are used to treat acute pancreatitis, but their effectiveness remains unclear. We performed a meta-analysis to determine whether treatment with protease inhibitors reduces overall mortality or morbidity from acute pancreatitis. METHODS: Articles of randomized controlled trials evaluating effects of protease inhibitors for acute pancreatitis were retrieved by systematically searching Medline, the Cochrane Library and Journal@ovid databases published from January 1966 through December 2003. References of review articles were also searched manually. The main outcome in interest was the overall mortality rate from acute pancreatitis. RESULTS: Ten studies met the inclusion criteria. Treatment with protease inhibitors did not significantly reduce the mortality rate from acute pancreatitis (pooled risk difference, -0.03; 95% confidence interval, -0.07 to 0.01). Subgroup analyses showed that treatment with protease inhibitors significantly reduced the mortality rate in patients with moderate to severe pancreatitis (pooled risk difference, -0.07; 95% confidence interval, -0.13 to -0.01) as defined by mortality rate in the control group (control mortality rate > 0.10). The decrease in mortality rate was not significant in mild pancreatitis (pooled risk difference, 0.00; 95% confidence interval, -0.04 to 0.05). CONCLUSIONS: Treatment with protease inhibitors does not significantly reduce the mortality in patients with acute or mild pancreatitis, but may reduce the mortality in patients with moderate to severe pancreatitis." I was in the ER recently for severe acute pancreatitis. They took blood and when it came back said something about my liver... the nurse said that acute pancreatitis is diagnosed by that result. Unfortunately I was completely doped up on morphine and gravol and don't remember exactly what she said. Is the liver always affected in acute pancreatitis? I can't find anything about this online. What exactly would the blood tests have shown? I'm just wanting to read up more on it, as I'm weighing my options on gallbladder surgery (MRI and ultrasound showed no stones but the doctor feels that that's most likely what it is). He was on strong antibiotics for past 2-3 months and was being treated for intra-abdominal abscess. The docs stopped the antibiotics 3 days ago, and the last blood test (yesterday) shows:
ALT at 907 (increased from 739 on the last day on antibiotics)
AST at 240
Total billirubin at 64
His hepatitis B & C tests came back negative and the latest CT scan did not mention anything abnormal with the liver/pancreas.
He suffered acute pancreatitis in March 2010 but has recovered from it. Except for occasional dark yellow urine, he shows no other symptoms. What else could be wrong with him?Last CT scan was done on 8th July. Antibiotics were stopped on 9th and ALT level on that day was 739. He is currently on vitamins only, mainly Jetepar,Vitamin C, B Co, folic acid, Neurobion. He is also taking multivitamin and selenium tabs for the past 4 months. All these are under doctor's prescription.
He also mentions itchiness around the left side of his upper abdomen (from back to right under the left arm).

I have been having what I now know are acute pancreatitis attacks for three months. It got so bad that it hurt to breathe and I gave in and had to go to the ER in the middle of the night. They took blood and said my pancreas is swollen and something to do with my liver (I dunno, they said that they can tell it's pancreatitis because of something to do with the liver?). They did an ultrasound but no gallstones showed up. They did an MRI today and it came back normal as well. I do not drink, smoke, or do drugs, at all. The attacks come on randomly and have been getting longer and worse. One doctor here says he would suggest I have my gallbladder removed tomorrow anyway and that there's probably just really tiny stones, but has no proof of it. The other doctor says he would wait and see if it happens again. How likely is it really to be gallstones? If they were so tiny that the MRI didn't pick them up, wouldn't they have passed on their own? Wouldn't they have been collecting at this point to cause so many problems, so that they would show up? Any advice?It isn't a serious medical question. I have had multiple opinions from doctors now. I am just hoping to hear from others who have gone in for pancreatitis and the cause was not determined. Just like I'm asking my friends and family for their opinions, I'm hoping to hear from others with similar experiences. I do not want to have surgery when the doctor says he "has no proof" and the other doctor disagrees with him but can't give me ANY cause himself. They do not know the cause but they say it could have been that I drank excessively during a graduation party 4 days prior. My question is it normal for pancreatitis brought on by a weekend of drinking to take 4 days to show symptoms (ie abdominal tenderness)? Again the pain was mild and only felt when I laughed or coughed. Also my doctor said that I can only drink one beer at a time now for the rest of my life...I'm 26 years old.

He's been throwing up a little bit recently, just a bit of yellowish stuff..
But now, we're finding his pee all over the house. He's house trained, so he always used to go outside. We got him from the pound so we didn't train him ourselves. Just a few hours ago, he walked into my doorway and just stood there and peed. I started trying to get him outside and he stood for a little bit looking really confused, then he snapped out of it and ran outside. It was like he couldn't control it, or didn't realize what he was doing. Earlier this morning I left the living room for a minute and the next thing I know there's pee all over the ottoman. When he peed in the house before (a long time ago), we'd think it was because he was angry at us, because he never did unless we'd been away all day. He loves company, and on the days that we're out most of the day, he gets angry. He's very very energetic, greets us at the door and everything. He loves to eat, but we're careful not to overfeed him.
I'm really worried about him, seeing that he's been with us a while and is getting old.
If it helps any, he's a maltese/terrier mix, probably about 7 or 8. He's had acute pancreatitis before, but it cleared up after just a little bit.

Nearly a month ago, my husband had surgery to remove most of his pancreas & his gallbladder after acute hemorragic pancreatitis. He developed abcesses & had another surgery Saturday to clean everything out, including a bit of dead pancreas. He had his first bowel movement yesterday & it was full of blood, as has been the horrible diarrhea he had all day today. His hemoglobin is being checked every 8 hours & he's had to have 2 or 3 transfusions. A gastroscopy, unsurpringly, showed nothing & he'll have more tests tomorrow. What is the likelihood of such massive bleeding being caused by anything other than the surgeon cutting/nicking a vessel?

When I was 11 years old I was rushed to hospital and diagnosed with acute pancreatitis. I had never had any alcohol in my life so that one is off the list. I had to have a nasogastric tube in me for 3 and a half days while on a IV and i wasn't allowed to be put back onto liquids for another two weeks. when i kept the water down they moved me onto food and eventually they let me out. in outpatients, i saw every specialist that i could and i had ever scan and test available including that for gallstones and billias slat, and its neither of them. I had numerous MRCP scans and ultrasounds, both showed nothing, my family and i also had a test done to see if this was hereditoy in my family or if i could give it to my children should i ever have any. the results said no. In the end they put me down to an UNKNOWN CAUSE. 4 years later (just a few weeks ago) I got acute pancreatitis again and then had a relapse after that, i had to go through the whole traumatic experience again with tubes, scans and an IV and i am now back at home recovering. in a few weeks i will have another MRCP scan. HOWEVER I NEED YOUR HELP
i know that these scans will just come up with nothing again, but i am very anxious to find out what causes this, anything at all apart from the mentioned above. If there are any nurses or doctors out there who could shed some possible light on this matter i would be very grateful. Right now i have been put on 15mg of lansoprazole to have everymorning before food. and i am on a BLAND diet. if that helps. also anyone who has been or is in a similar situation then please feel free to email me =]Also thanks to anyone who replys with sensible replys. I was hospitalized about 3 years ago with an episode of acute pancreatitis. About 6 days ago, I finally drank again and had 1 beer over the course of about 2 hours. Since then, I've had a persistent very dull pain in the general area under my ribcage, right above my belly button. After the pain subsides, the area feels bruised to the touch. I also have a lot of gas and acid reflux. My stool is normal, eating habits are normal. I'm just really hoping this isn't pancreatitis again. I was only 16 when I had my first attack, and I wasn't even a heavy drinker back then either (although it was caused by having 3 beers in one night).
So what do you guys think? I already set an appointment to see my doctor soon. I'm just freaking out here and am really hoping it's not pancreatitis again. I'm hoping it's just acid reflux, which I also have a history with, although my acid reflux usually affects my upper chest and not my abdomen.

Since June '07, I've had stomach pain the reoccurs in the same spot. It's the upper left side of my belly button. It's like a crampy, throbbing pain. The intensity of the pain varies. It gets pretty bad sometimes.
In July '09, I had to go to the hospital, after consuming 6 sparks(flavored beer). It turned out, I had acute pancreatitis. I quit drinking till September '09, then one night, I broke down and had about 7 shots of Captain Morgan Rum. I had to go back. I had acute pancreatitis again. I havnt had alcohol since then. But now, I have stomach pain every day. And pain medications make me sick to my stomach. I take them after eating a meal, and I drink plenty of water. But it always seems to make me feel very nauseous, and sick. I end up throwing up. And then I feel so fatigued for the rest of the day, sometimes even the next day or two. My doctor thinks it's just acid in my stomach. So she prescribed me rantidine. I've been taking it for a month. It helps with heartburn, but my stomach still hurts.

Chronic pancreatitis diagnosed fifteen months ago, Dx changed a few weeks later to acute pancreatitis. Subsequently zero alcohol (previously drank two drinks each evening), stools more frequent and often liquid or very soft. The main problem is a vast amount of flatus. Is this the result of pancreatitis or should I be looking for something else? I take Ultrase 2 tid with meals, plus Welchol, Zetia, and Niaspan for lipid control. Pain was never severe, have had none for many months. Why so much gas, and can I reduce it?

I've been asking a lot about this condition lately, and I'm sorry if it is starting to get old, but at 9pm at night, you don't have a lot of other sources to go to!
How should acute pancreatitis be treated? The reason I ask is because I have been recently diagnosed with it, however not treated, and my GP thinks I have been clinically neglected on the hospitals part, and even gave me a statement I could use if I wish to follow up a complaint for Clinical Negligence.
GP says given my medical tests, results and symtoms, that I should have been in hospital, on an intraveinous drip given fluids (Due to the dehydration caused by not being able to keep anything down- or in), and surgical intervention or at least consultation should have taken place.
Instead I was sent home with no medication, doctor at the hospital di not beleive it was pancreas related as (In his exact words) "It can not be the pancreas, as you do not drink alcohol or do drugs".
He said he consulted with the surgeon on call, but when asked what the sugeon said he would not say anything. He did not give us the surgeons name, and when we requested to see him, he said he was too busy.
Anyway, all of that aside, GP (Who has worked many E.Rs) says treatment should have been initiated based on my results, but obviously it wasn't.

GP is outraged and sends patient back to hospital for treatment.
Blood tests today reveal initial inflammation has subsided but symptoms still persist, since bloods are OK it is decided no treatment necessary and sent home again.
Was the first event clinical negligence?

I took him to the vet and after a few tests they said they didn't know what was causing it and sent us home with some meds. Two days later he had no improvement so I took him back and they hospitalized him. Three days later I was able to pick him up but before I got there they called and told me he had developed a limp in his left back leg and that it was probably caused from his nausea meds. They changed his medication and told me that I should notice his limp improve after a couple days with it being gone in a week.It's been a week and I have seen no improvement at all. I took him back to the vet and they tested him for diabetes which came back negative and told me they suspected he had had an acute pancreatitis. They still insists that the meds caused his limp. After a week shouldn't they be out of his system? Should I take him to another vet? He doesn't seem to be in any pain but I'm worried something more is going on here, Honestly my first thought was that maybe they dropped him during a feeding( they had to force feed him) and hurt his leg. Any advice would be greatly appreciated. Thanks

Ok I'll try to be brief......
I am studying abroad right now and do not have access to my doctor until next summer. Im 22 btw.
I have not drank since last summer (7 months) and its been REALLY tough. Ive made a lot of new friends here but the fact that I cant drink SERIOUSLY injures my social life. (I cant just go to bars and sit around while everyone gets drunk---ive done this a couple times and its just weird for everyone).
So anyways my acute attack was relatively mild considering what Ive read about the disease---I was not hospitalized or put on any IVs. I just took painkillers for 1 day and thats it. I just was on a super strict diet for about 5 days. Then it went away.
So my questions.....and I btw I WILL WAIT TILL I SEE MY DOCTOR AGAIN BEFORE I START DRINKING AGAIN. Im hoping he will say something positive, but I just want some general questions answered.
Will I be able to ever drink again?? And even if I can....Will I be able to get DRUNK again?
If my doctor says no....
Is there any kind of medication or even procedure which will allow me to drink/get drunk again?
I just need some INFORMATION. I already checked some sites like WebMD and stuff.......but if you know any more helpful links post them here.
I REALLY need to know......not being able to drink is absolutely destroying my social life.....and no one is availible to me to give me advice.........SO PLEASE GIVE ME SOME INFO!!!!
Whatever the case is I WILL NOT TOUCH A DROP until I see my doctor........This is just for general information.

hi I was diagnosed with acute pancreatitis. over the weekend and spent a day and a half in a hospital receiving ivs and only drinking fluids. This was due to my alcohol use. I only drink socially but when I do I binge drink. Im not an alcoholic just a normal college kid. Im having a hard time accepting the fact that I may be able to ever drink again but is there anyway I possibly could if I closely monitor my alcohol use and drink in moderation and cut back a lot. Obviously my health is #1 and I am planning on taking an indefinite time off of drinking until I fully recover from this. It sucks bc my friends 21st bday is in 2 weeks and me and a bunch of friends were planning on going to celebrate in vegas for spring break in 1 month. The doctor said I recovered really fast bc I had been taking good care of my body otherwise (diet) etc. So will there be any possible way I can come back or is alcohol a done deal in my life?

I'm just wondering if your enzymes are 17,000 (should be under 100) shouldn't you be in the hospital? I have a friend who says she did outpatient labs & that is how high her enzymes were. Can't you die from pancreatitis? When my levels were 7,000 there was no way I could deal with the acute attack at home. I could barely walk or talk let alone drive myself to the hospital/doctors office. My doctors didn't really give me a choice but to go the hospital so it seems strange to me that they sent her home. In october i was admitted into hospital with acute pancreatitis caused by gallstones. I was in hospital for 8 days, in which i had my gallbladder removed. The doctor told me that i could suffer pancreatitis again.
On wednesday i started to get the worst pain ever, but this time in my lower abdomen. This pain has not gone away, and i still have it now :( My stomach is very very tender to touch, and the pain is in my lower stomach and radiates round to my back but does move up around my ribs (which is where i had pain when i did have pancreatitis) My stomach feels very swollen, and the pain only eases when i sit forward, but really hurts when i stand! Im just really scared that i may have pancreatitis again, im only 24 and have 3 young children so this is all i need. About 7 weeks ago, I went into the hospital with what I thought was food poisoning, but turned out to be 2 things. The first being a perforation in my duodenum, the 2nd being mild acute pancreatitis.
Had surgery to repair the perforation, then was NPO for the remaining 3 weeks I was in the hospital, on TPN... I was released 3 weeks ago, have been NPO since on TPN at home. I admit I've cheated a little bit, had some grilled chicken breast, some soup, clear liquids, etc.. but it's not like I'm eating 3 meals a day or anything...
So, lipase in the hospital was in the 780s range.. I was released with it running in the 550s... A week later they drew blood and I was down in the 220's, then 178, then 108, and now 145... I had a CT 2 weeks ago showing the pancreatits was pretty much gone, no inflammation or anything left.
The Dr. seeing me right now, is my surgeon, and my appointments with him are very brief, and he is usually very short with me... and sometimes he just calls me and says more TPN, we'll try again in a week.
So my question is, first off.. what would be good lipase levels? According to quest diagnostics website (they are also the ones that do my labs) a normal lipase level for someone my age (26) is anything under 140. I got that information from http://www.questdiagnostics.com/kbase/topic/medtest/hw7976/results.htm - is this accurate? My amylase and the rest of my labs are great.
My Dr. wants me under 100 before he can feel like we can stop the TPN, which by the way is a pain in the ass, and it keeps me from going to work.
Given the results of my labs, and what the brief period of time I get advise from my Dr., should I get a 2nd opinion, or should I just rough it out?

Long story short...my brother is an alcoholic. He is living with my husband and I temporarily so I'm seeing firsthand his actions and the consequences.
To sum it up, I know my brother was hospitalized shortly with acute pancreatitis last year...and from what I can gather from docs, his liver functions tests have come back very high ever since. He was told to stop drinking but never did.
Back then, he suffered from pain in his stomach and had chronic nausea and vomiting. Now, however, he is having chronic nausea and vomiting several times a week but says he feels absolutely NO pain. I have read that as pancreatitis progresses, the pain can actually subside. Anyone have experience with this as being true? He refuses to go to a doc..although this evening I told him he must if he wants to stay with us for the time we agreed upon. I think he is finally willing to go now but it wouldn't surprise me if he bailed on that plan and moved out.
By the way, I have pleaded with him to stop drinking and get some help but his denial is unbelievable. He actually spends a day vomiting and then goes right back to drinking the following day and actually WONDERS why he continues to vomit. THAT is how deeply in denial this man has fallen.
Please share your knowledge and experience if you have it (regarding the pancreatitis with no pain symptoms).
My husband, ironically, is recovering from a similar path in his life (with drinking) but is doing great currently so I feel like I've been there already once and really don't enjoy seeing someone else in my life go down the same road.

Well that's what the hospital thought I had, I discharged myself before my blood work came back.
I'm female and 18 and am not 100% sure I actually had pancreatitis. I had my last drink on Sunday morning about 4am? And started feeling really unwell about 9pm Monday night. Started with a dull back ache and then quite a severe upper adominal pain, I couldn't breathe. I was just starting to get a really bad cold aswell. I made myself sick and then was sick a couple of times without making myself when I got to the hospital. They did scans and it showed that everything was fine and healthy, they didn't even mention my pancreas so I'm assuming it wasn't swollen, they did a chest xray that came back healthy aswell. I was in for 3 days and they wanted me to stay at least another 2.
The Doctor I spoke to said that "Next time you drink make sure to line your stomach drink some milk or take some gaviston and stick to your recommended allowance of units." But I've been reading some internet sites and it says that I can never drink again?
What's the deal?

So... my brother has been an alcoholic for a little over 12 years. He's had multiple cases of acute pancreatitis and is currently in remission from esophageal cancer. As far as I know he still drinks. My brother is currently 36.
My sister, last we spoke about it, weighs an excess of 445 pounds. She has always been large but, according to our last conversation, she has gained 105 pounds in the last 8 months. She'll turn 32 next month.
Both of them seem reserved to death. I love them with all my heart and wish I could do something (ANYTHING!) for them. Our mother died of an alcohol poisoning just over ten years ago. We never knew our father.
What can I do to stave off this seemingly inevitable tragedy? I don't want to be alone in this world...
