"OBJECTIVES: Protease inhibitors are used to treat acute pancreatitis, but their effectiveness remains unclear. We performed a meta-analysis to determine whether treatment with protease inhibitors reduces overall mortality or morbidity from acute pancreatitis. METHODS: Articles of randomized controlled trials evaluating effects of protease inhibitors for acute pancreatitis were retrieved by systematically searching Medline, the Cochrane Library and Journal@ovid databases published from January 1966 through December 2003. References of review articles were also searched manually. The main outcome in interest was the overall mortality rate from acute pancreatitis. RESULTS: Ten studies met the inclusion criteria. Treatment with protease inhibitors did not significantly reduce the mortality rate from acute pancreatitis (pooled risk difference, -0.03; 95% confidence interval, -0.07 to 0.01). Subgroup analyses showed that treatment with protease inhibitors significantly reduced the mortality rate in patients with moderate to severe pancreatitis (pooled risk difference, -0.07; 95% confidence interval, -0.13 to -0.01) as defined by mortality rate in the control group (control mortality rate > 0.10). He's been throwing up a little bit recently, just a bit of yellowish stuff.. But now, we're finding his pee all over the house. He's house trained, so he always used to go outside. We got him from the pound so we didn't train him ourselves. Just a few hours ago, he walked into my doorway and just stood there and peed. I started trying to get him outside and he stood for a little bit looking really confused, then he snapped out of it and ran outside. It was like he couldn't control it, or didn't realize what he was doing. Earlier this morning I left the living room for a minute and the next thing I know there's pee all over the ottoman. When he peed in the house before (a long time ago), we'd think it was because he was angry at us, because he never did unless we'd been away all day. He loves company, and on the days that we're out most of the day, he gets angry. He's very very energetic, greets us at the door and everything. He loves to eat, but we're careful not to overfeed him. I'm really worried about him, seeing that he's been with us a while and is getting old. If it helps any, he's a maltese/terrier mix, probably about 7 or 8. He's had acute pancreatitis before, but it cleared up after just a little bit. Can anyone help? Nearly a month ago, my husband had surgery to remove most of his pancreas & his gallbladder after acute hemorragic pancreatitis. He developed abcesses & had another surgery Saturday to clean everything out, including a bit of dead pancreas. Okay so my four year old dog, Teddy, has been through so much from being diagnoised with epilepsy at 6 months of age to now having pancreatitis. When I was 11 years old I was rushed to hospital and diagnosed with acute pancreatitis. I had never had any alcohol in my life so that one is off the list. I had to have a nasogastric tube in me for 3 and a half days while on a IV and i wasn't allowed to be put back onto liquids for another two weeks. when i kept the water down they moved me onto food and eventually they let me out. in outpatients, i saw every specialist that i could and i had ever scan and test available including that for gallstones and billias slat, and its neither of them. I had numerous MRCP scans and ultrasounds, both showed nothing, my family and i also had a test done to see if this was hereditoy in my family or if i could give it to my children should i ever have any. the results said no. In the end they put me down to an UNKNOWN CAUSE. 4 years later (just a few weeks ago) I got acute pancreatitis again and then had a relapse after that, i had to go through the whole traumatic experience again with tubes, scans and an IV and i am now back at home recovering. in a few weeks i will have another MRCP scan. HOWEVER I NEED YOUR HELP i know that these scans will just come up with nothing again, but i am very anxious to find out what causes this, anything at all apart from the mentioned above. If there are any nurses or doctors out there who could shed some possible light on this matter i would be very grateful. I was hospitalized about 3 years ago with an episode of acute pancreatitis. About 6 days ago, I finally drank again and had 1 beer over the course of about 2 hours. Since then, I've had a persistent very dull pain in the general area under my ribcage, right above my belly button. After the pain subsides, the area feels bruised to the touch. I also have a lot of gas and acid reflux. My stool is normal, eating habits are normal. I'm just really hoping this isn't pancreatitis again. I was only 16 when I had my first attack, and I wasn't even a heavy drinker back then either (although it was caused by having 3 beers in one night). So what do you guys think? Since June '07, I've had stomach pain the reoccurs in the same spot. It's the upper left side of my belly button. It's like a crampy, throbbing pain. The intensity of the pain varies. Chronic pancreatitis diagnosed fifteen months ago, Dx changed a few weeks later to acute pancreatitis. Subsequently zero alcohol (previously drank two drinks each evening), stools more frequent and often liquid or very soft. The main problem is a vast amount of flatus. I've been asking a lot about this condition lately, and I'm sorry if it is starting to get old, but at 9pm at night, you don't have a lot of other sources to go to! How should acute pancreatitis be treated? The reason I ask is because I have been recently diagnosed with it, however not treated, and my GP thinks I have been clinically neglected on the hospitals part, and even gave me a statement I could use if I wish to follow up a complaint for Clinical Negligence. GP says given my medical tests, results and symtoms, that I should have been in hospital, on an intraveinous drip given fluids (Due to the dehydration caused by not being able to keep anything down- or in), and surgical intervention or at least consultation should have taken place. Instead I was sent home with no medication, doctor at the hospital di not beleive it was pancreas related as (In his exact words) "It can not be the pancreas, as you do not drink alcohol or do drugs". He said he consulted with the surgeon on call, but when asked what the sugeon said he would not say anything. He did not give us the surgeons name, and when we requested to see him, he said he was too busy. Anyway, all of that aside, GP (Who has worked many E.Rs) says treatment should have been initiated based on my results, but obviously it wasn't. I'm still having the same symptoms, so obviously it's not just a bug that's taking it's sweet time to pass. How should pancreatitis be treated normally? My cat became ill two weeks ago;he stopped eating and drinking and was vomiting. I took him to the vet and after a few tests they said they didn't know what was causing it and sent us home with some meds. Two days later he had no improvement so I took him back and they hospitalized him. Three days later I was able to pick him up but before I got there they called and told me he had developed a limp in his left back leg and that it was probably caused from his nausea meds. They changed his medication and told me that I should notice his limp improve after a couple days with it being gone in a week.It's been a week and I have seen no improvement at all. I took him back to the vet and they tested him for diabetes which came back negative and told me they suspected he had had an acute pancreatitis. They still insists that the meds caused his limp. After a week shouldn't they be out of his system? Should I take him to another vet? Ok I'll try to be brief...... I am studying abroad right now and do not have access to my doctor until next summer. Im 22 btw. I have not drank since last summer (7 months) and its been REALLY tough. Ive made a lot of new friends here but the fact that I cant drink SERIOUSLY injures my social life. (I cant just go to bars and sit around while everyone gets drunk---ive done this a couple times and its just weird for everyone). So anyways my acute attack was relatively mild considering what Ive read about the disease---I was not hospitalized or put on any IVs. I just took painkillers for 1 day and thats it. I just was on a super strict diet for about 5 days. Then it went away. So my questions.....and I btw I WILL WAIT TILL I SEE MY DOCTOR AGAIN BEFORE I START DRINKING AGAIN. Im hoping he will say something positive, but I just want some general questions answered. Will I be able to ever drink again?? And even if I can....Will I be able to get DRUNK again? If my doctor says no.... Is there any kind of medication or even procedure which will allow me to drink/get drunk again? I just need some INFORMATION. hi I was diagnosed with acute pancreatitis. over the weekend and spent a day and a half in a hospital receiving ivs and only drinking fluids. This was due to my alcohol use. I only drink socially but when I do I binge drink. Im not an alcoholic just a normal college kid. Im having a hard time accepting the fact that I may be able to ever drink again but is there anyway I possibly could if I closely monitor my alcohol use and drink in moderation and cut back a lot. Obviously my health is #1 and I am planning on taking an indefinite time off of drinking until I fully recover from this. I'm just wondering if your enzymes are 17,000 (should be under 100) shouldn't you be in the hospital? I have a friend who says she did outpatient labs & that is how high her enzymes were. In october i was admitted into hospital with acute pancreatitis caused by gallstones. I was in hospital for 8 days, in which i had my gallbladder removed. The doctor told me that i could suffer pancreatitis again. On wednesday i started to get the worst pain ever, but this time in my lower abdomen. This pain has not gone away, and i still have it now :( My stomach is very very tender to touch, and the pain is in my lower stomach and radiates round to my back but does move up around my ribs (which is where i had pain when i did have pancreatitis) My stomach feels very swollen, and the pain only eases when i sit forward, but really hurts when i stand! Im just really scared that i may have pancreatitis again, im only 24 and have 3 young children so this is all i need. Hello, About 7 weeks ago, I went into the hospital with what I thought was food poisoning, but turned out to be 2 things. The first being a perforation in my duodenum, the 2nd being mild acute pancreatitis. Had surgery to repair the perforation, then was NPO for the remaining 3 weeks I was in the hospital, on TPN... I was released 3 weeks ago, have been NPO since on TPN at home. I admit I've cheated a little bit, had some grilled chicken breast, some soup, clear liquids, etc.. but it's not like I'm eating 3 meals a day or anything... So, lipase in the hospital was in the 780s range.. I was released with it running in the 550s... A week later they drew blood and I was down in the 220's, then 178, then 108, and now 145... I had a CT 2 weeks ago showing the pancreatits was pretty much gone, no inflammation or anything left. The Dr. seeing me right now, is my surgeon, and my appointments with him are very brief, and he is usually very short with me... and sometimes he just calls me and says more TPN, we'll try again in a week. So my question is, first off.. what would be good lipase levels? According to quest diagnostics website (they are also the ones that do my labs) a normal lipase level for someone my age (26) is anything under 140. My Dr. wants me under 100 before he can feel like we can stop the TPN, which by the way is a pain in the ass, and it keeps me from going to work. Long story short...my brother is an alcoholic. He is living with my husband and I temporarily so I'm seeing firsthand his actions and the consequences. To sum it up, I know my brother was hospitalized shortly with acute pancreatitis last year...and from what I can gather from docs, his liver functions tests have come back very high ever since. He was told to stop drinking but never did. Back then, he suffered from pain in his stomach and had chronic nausea and vomiting. Now, however, he is having chronic nausea and vomiting several times a week but says he feels absolutely NO pain. I have read that as pancreatitis progresses, the pain can actually subside. Anyone have experience with this as being true? He refuses to go to a doc..although this evening I told him he must if he wants to stay with us for the time we agreed upon. I think he is finally willing to go now but it wouldn't surprise me if he bailed on that plan and moved out. By the way, I have pleaded with him to stop drinking and get some help but his denial is unbelievable. He actually spends a day vomiting and then goes right back to drinking the following day and actually WONDERS why he continues to vomit. THAT is how deeply in denial this man has fallen. Please share your knowledge and experience if you have it (regarding the pancreatitis with no pain symptoms). My husband, ironically, is recovering from a similar path in his life (with drinking) but is doing great currently so I feel like I've been there already once and really don't enjoy seeing someone else in my life go down the same road. Well that's what the hospital thought I had, I discharged myself before my blood work came back. I'm female and 18 and am not 100% sure I actually had pancreatitis. I had my last drink on Sunday morning about 4am? And started feeling really unwell about 9pm Monday night. Started with a dull back ache and then quite a severe upper adominal pain, I couldn't breathe. I was just starting to get a really bad cold aswell. I made myself sick and then was sick a couple of times without making myself when I got to the hospital. They did scans and it showed that everything was fine and healthy, they didn't even mention my pancreas so I'm assuming it wasn't swollen, they did a chest xray that came back healthy aswell. I was in for 3 days and they wanted me to stay at least another 2. The Doctor I spoke to said that "Next time you drink make sure to line your stomach drink some milk or take some gaviston and stick to your recommended allowance of units." If someone is able to help with these questions I would be very grateful. The doctors are of some help, but very little. I am just looking for answers as to why this is happening to my dad. My dad was admitted to the hospital Saturday night with acute Pancreatitis. By Tuesday morning he had become extremely disoriented and confused- didn't understand where he was. He bagan pulling out his IV's and becoming very agitated. He was on Dilluadid and Adivant to keep him comfortable. Tuesday morning he was transferred to the ICU because the nurse noticed him struggling while breathing. The doctor's had to put him in restraints so that he would not pull out the new cathader under his collar bone administering his drip. The doctors are now concerned my dad has ARDS, or Pneumonia. I realize this is a test by exclusion but they seem more than convinced based on his extrememely low oxygen level of 50% that he does in fact have ARDS. He is on a respirator right now and heavily sedated so that he will not hurt himself becoming agitated while the respirator is in. My questions are, how does Pancreatitis actually induce ARDS development? And can his oxygen level possibly indicate his improvement with the ARDS and possibly indicate survival upon its improvement? I have looked up ARDS for the past day and feel I know a lot about the disease but there are some questions that the internet can't answer, and that the doctor doesn't have time to answer. Any help is appreciated. This disease is life threatening with a so-so survival rate. Also my dad is a 63 year old smoker. Smokes about half a pack a day and drinks about two drinks per night (despite the doctor's implication that he is an alcoholic- definintely had to read him the riot act on that one). After deciding it probably wasn't JUST the alcohol that could have cause these "withdrawal" symptoms of disorientation and confusion they turned to Benzodiazipine withdrawals since my dad took 1-2mg of xanax each night before bed. First I will say that my lab is only 2 & 1/2yrs old. So about 4 days ago my black lab got sick (not uncommon for her she has a bad GI system) we figured she ate to fast. Anyways, when I got up in the morning she was not acting her self. She didnt want to eat (even people food) or drink. Later on in the day she began doing this posturing stance where she would just walk around and then stop and stand there forever. She would not sit down or lay down, and she just would whine when you tried to make her get on furniture to rest or even lay down on the floor. We took her to the vet and had her checked out. But when we got to the vet she acted all normal and fine. They did express her anal glands as they were full, but they couldnt pinpoint the pain. They thought maybe pancreatitis but that was ruled out through lab work. So we brought her home and she continued to pace and walk around and not sit down. However, she started to eat and drink and was going to the bathroom. But she still did not want to play. The vet said if there was no improvement by the next day to bring her back. So we did bring her back and again she acted normal when we got to the vet. So they did radiographs, xrays, more blood work, etc and could not find anything abnormal. (There was like .2 increase on her white blood cells which honestly didnt mean a single thing). So the only thing the vet could tell me was possibly a soft tissue injury was causing her pain (due to the WBC count). They sent us home with rimadyl and after taking it she started to be alright. She has been on rimadyl before so I know it doesnt affect her badly (as I know most of you have probably seen the bad reports about pfizer and dog deaths due to rimadyl). Anyways, I decided to take her off it after only 2 days because she was acting decently like her self again. She had her last dose yesterday (wed 11/4/09) at 4:30-5ish. And so far today she is eating, wanting to play, relaxing, getting up on the furniture without us prying her to do so, and all body systems seem to be working ok. I am just wondering if anyone knows what could have possibly caused her to get this acute pain that cannot be explained, also if you ever had this happen to your dog (and did they get better), and if you know how long it takes for rimadyl to be out of the system...therefore I can see if it acutally is my dog getting better or the meds just masking her pain. She is not overweight she is actually right where she needs to be weight wise. I am thinking it could have been a number of things. I know it wasnt the smartest to take her off the rimadyl before the prescribed time but I just thought she was getting better so why keep a dog on soemthing that could cause further complications when she is already ok. At 5pm today it will be 24hrs since she had her last dose, so hopefully by then we will be able to see if it was the rimadyl or herself getting better on her own. The vet did a very good check on all the tests, and they actually had a second vet look at all of them just as a second opinion...neither could find anything! Strange. My husband awoke in the wee hours this morning with crushing pain right at the ribcage level and ended up vomiting a couple of times. We've been to the doctor and he thinks it's gastritis; he first tried having him swallow a topical anesthetic to be sure the pain wasn't radiating from the esophagus or upper portion of the stomach because of acid reflux (he just threw that up, and the pain wasn't helped at all). After making his diagnosis, he gave my husband some samples of Nexium as well as a couple of other prescriptions for nausea and such. No more than 10 minutes after he took the Nexium, he felt almost perfect; a little shaky in the stomach still, but the pain was gone, as was the strong nausea. MY fear was that he had a gallstone attack, which of course would eventually result in surgery, and he isn't insured yet. We can't figure out what would have brought on the gastritis, unless this sounds like a recipe for it: that night, we went out to eat, and he had a burger. Later on, we had fast food, which was a fried chicken sandwich. He had SEVERAL Crown Royal and Cokes that night. And in general he tends to drink a lot of soda. Does all this sound like it could cause at least acute gastritis? I was in hospital for a week, on IV and morphine. I was released and gradually built up my appetite again. It was confirmed to not have been caused by gal stones, and most likely instead, alcohol. They increased in the past year to the point where he has pain every day, several times and the pain has intensified since he had his last real bad attack 3 weeks ago and went to the hospital for 3 days. The dr. ran tests, said his blood pressure was real high. and his pancreas was inflammed greatly. he was throwing up bile the night before and the next day when he went into the hospital. Dr. said they could not do anything else other than ease the pain and run tests and they sent him home in pain with pain pills which put him to sleep most of the day. He was a mild drinker. And has vowed to stop drinking after this attack. So that is good. He has twisted intestines and had his gallbladder removed when he was first diagnosed with pancreatitis. He does not have diabetes or pancreatitis cancer. He does have bouts of weakness, feeling tired, has hot/cold spells at night as if his blood pressure is fluctuating. Sometimes he has a hard time catching his breath and it scares me because I worry he is going to die, but I place a fan in front of his face and he gets better after awhile and breathes normally. During the day he sweats alot when it is hot and needs lot of air to stay cool. I hear that ceder berrys are good for the pancreas. Lipase is good, but I am concerned about that because if pancreatitis secretes lipase which eats at the pancreas, why would one want to take that. Is there something to calm down the pancreas and keep it from digesting itself. I am looking for natural herbal supplements that work. I got him some pancre-ease tablets and something I cant think of the name but it helps regulate the blood pressure. Just got some cedar berry tablets.
